To keep the family feel the manager will visit each client herself prior to support commencing to ensure that the correct personalities and skill sets are matched.
Once contacted, PRIME HOME CARE will undergo an on feild assesment and shortly we will arrange for an assessor to visit you and review your care needs. The assessment is free of charge and is required in order to improve and to optimize our services.
A list of services and fees will be sent for you, and informed consent of care agreement should be signed before starting our plan.
Once you are ready to commence your care services, our home care specialists can work with you to set lifestyle goals and create a personalised plan that helps you reach them.
